If you have faced discrimination or encountered barriers because of your disability, Freedom Resource Center can help. This form lets you share what happened so we can review your experience, connect you to resources, and explore possible next steps together.
When to Use this Form
- You were denied services in education, housing, or employment because of your disability
- You were treated unfairly or excluded from participating in an activity or service
- You experienced a lack of accessibility in a public or business setting
- You want guidance on filing a formal complaint
How to Complete the Form
- Online: Fill out the form below and click “Submit”
- In Person: Visit a Freedom office to share your experience with an advocate
- By Phone: Call your local office and we can schedule a phone appointment to complete the form with you
After Form is Completed
- Your information will be reviewed by Freedom Resource Center staff
- We may contact you for more details or documentation
- We’ll provide information on your rights
- Submitting this form is the first step to exploring your options
Your submission will be kept confidential and will only be shared with staff involved in responding to your inquiry.
What to do if you’ve experienced discrimination.
State and federal laws like the Minnesota or North Dakota Human Rights Act, The Americans with Disabilities Act, and the Fair Housing Act are in place to protect people with disabilities from illegal discrimination.
