What This Service Is
We support people with disabilities in building the lives they want whether that means transitioning from nursing homes or other institutions back into the community, staying in their homes and avoiding institutional placement, or helping youth move confidently into adulthood.
Who It’s For
People of any age who are:
- Leaving an institution and moving into community living of your choice
- At risk of entering an institution
- Youth transitioning from high school
